About

The Indigenous Complex Care Housing program is a specialized initiative designed to support Indigenous individuals living with complex mental health and substance use challenges. This housing model provides a home-like environment that includes health, mental health, and social support services tailored to the unique needs of Indigenous communities. 

This program is built on a person-centred, culturally safe, and trauma-informed approach. It integrates health services into housing to help clients build a sustainable foundation for achieving their personal goals.

  • Ensures that individuals are safely housed, fed, and receive on-site medical and cultural support.

  • Indigenous-specific complex care supports are connected to the individual, not tied to a specific location—this helps maintain ongoing stability.

  • Robust service delivery is achieved through:

    • Coordinated access and assessment

    • Integrated coordinated access tables

    • Indigenous harm reduction teams

    • Intensive case management

    • Assertive community treatment teams

    • 24/7 fixed-site teams

Who can access these programs or services

Indigenous individuals aged 19 and older who are experiencing severe and persistent mental health and/or substance use challenges.

How to access services

Access is initiated through a referral application. Once a referral is submitted, our team will meet with the individual to complete an assessment. If the person meets all the program requirements, they will be enrolled in the service.
